At Skyreach Behavioral Health, we are committed to supporting individuals and families through a wide range of behavioral health services. Our programs include DUI Screening, Education, and Treatment, licensed by the State of Arizona and approved by ADOT, MVD, and the courts. We also provide therapy, Intensive Outpatient (IOP) services, anger management, domestic violence counseling, and child and family coaching.

We believe in empowering every member with the tools they need to grow, heal, and thrive, on the road and in life.

Our Mission is to Assess Our Members and Create a Treatment Plan That Best Fits Their Needs for Mental Health and Behavioral Health, Including Anxiety, Depression, Substance Use, Ptsd, Dui Services, Domestic Violence Services, and Many More. We Provide Care With Trained Staff Members in Behavior and Find Strategies to Achieve Our Mission. Our Priority is to Help Our Members Achieve a Well-balanced Life in Their Day-to-day Lives.

Daniel Fernandez, MBA: HSM

Administrator

Daniel’s leadership is grounded in personal experience and a deep commitment to transforming behavioral health care. Daniel has a focus on empathy, equity, and accessibility, he has created a supportive environment where individuals and families can build resilience, receive culturally responsive care, and thrive in their unique journeys.

Michael Israel, LCSW

Behavioral Health Professional

Michael brings over 13 years of experience in behavioral health and substance use support, guiding individuals through life’s challenges with compassion and expertise. He earned his Bachelor’s degree in Criminal Justice from UMass Boston and his Master of Social Work from Walden University. Michael’s therapeutic approach emphasizes self-compassion, forgiveness, and resolving inner conflicts as a way to create meaningful external change. He is dedicated to creating safe, supportive spaces where clients feel heard, empowered, and engaged in their growth. Outside of his professional work, Michael finds renewal in nature hiking several times a week, kayaking, and camping. His balance of clinical expertise and grounded presence enriches the care he provides to every individual and family he serves.

Andrea Hinton, APRN, PMHNP-BC

Psychiatric Nurse Practitioner

Andrea provides compassionate mental health care for teens and adults, with a holistic and collaborative approach. Allowing our members to find balance, healing, and wellness.

Andrea is a Psychiatric Mental Health Nurse Practitioner treating members ages 13 and up. Andrea’s approach is holistic, collaborative, and centered on truly listening to each member’s story. She specializes in depression, anxiety, PTSD, bipolar disorder, and ADHD. For those with a history of substance use, she provides safe non-stimulant options for ADHD care. “Together, we create a path toward balance and well-being.”

Fernanda

Behavioral Health Coach and Office Coordinator

Fernanda, a compassionate Behavioral Health Coach and Office Coordinator, is dedicated to working with members, creating a safe and supportive space where they can thrive. She promotes holistic wellness in their daily lives, focusing on life skills and advocating for each individual to ensure their success. Fernanda’s commitment to nurturing the whole person is at the heart of her approach. “You can’t let your failures define you. You have to let your failures teach you”

Mario

Behavioral Health Coach

Mario brings purpose and energy to every person he supports. He works closely with unhoused individuals and children, offering encouragement, structure, and meaningful connections. Mario served for several years as a Special Education Teacher, where he developed a deep understanding of individualized learning, behavior support, and the power of patient guidance. Outside of behavioral health, Mario coaches’ youth soccer, where he finds joy in watching growth unfold both on and off the field. His work reflects a deep belief in long-term progress and human potential. “Life’s race is not a sprint, it’s a marathon”

Behavioral Health Coach

Angelica brings over 15 years of experience in behavioral health and client advocacy to our team. She is passionate about connecting our members to essential resources, promoting community relationships, and ensuring every individual feels supported on their wellness journey. Angelica effectively serves diverse communities with compassion and expertise. Her dedication to empowering clients and building meaningful connections makes her an invaluable part of the team. “Every person deserves to feel seen, heard, and supported, I’m here to help make that happen.”

Therapist

Nicole is a therapist at Skyreach Behavioral Health with experience providing individual therapy and facilitating group sessions focused on substance abuse, domestic violence, anger management, parenting skills, and various mental health challenges. She offers a safe, supportive environment where clients can openly share and process their experiences. Nicole utilizes evidence-based interventions such as Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and other therapeutic approaches to help individuals navigate the difficulties of daily living and promote personal growth.

Delphine Valentin, APRN, PMHNP-BC

Psychiatric Nurse Practitioner

Delphine provides thoughtful, patient-centered psychiatric care for adolescents and adults, with an emphasis on safety, trust, and long-term wellness. She approaches mental health through a holistic and collaborative lens, taking time to understand each individual’s lived experience, strengths, and goals. Delphine treats a wide range of conditions including depression, anxiety, trauma-related disorders, mood disorders, and ADHD, and is especially mindful when supporting individuals with co-occurring substance use concerns. Her care style prioritizes shared decision-making, education, and realistic treatment planning, empowering members to feel confident and supported throughout their healing journey.

Kailynn Clark, FNP-C

Family Nurse Practitioner

Kailynn brings a compassionate, whole-person approach to integrated physical and behavioral health care. As a Family Nurse Practitioner, she focuses on preventive care, chronic condition management, and addressing the physical health needs that often intersect with mental health and substance use challenges. Kailynn believes strongly in meeting members where they are and creating a welcoming, judgment-free environment where health concerns can be addressed openly and collaboratively. Her practice emphasizes education, accessibility, and practical solutions that support long-term stability, wellness, and improved quality of life.
